Date : 7 July 2016 - 8 July 2016

Time : 9:00 a.m. - 12:00 noon, and 2:00 p.m. - 4:00 p.m

Venue : 22 Rose Street, Yau Yat Chuen, Kowloon Tong.

Documents required for Registration

1. Admission Slip;
2. HK Birth Certificate;
3. HKID Card or other documentary evidence showing the resident status in HK and date of birth (If the student’s HKID Card bears the symbol ‘C’ (for conditional stay) at the line below the date of birth, he/she should also bring along other valid travel documents.)(Copy);
4. the school reports of P5 & 6(Copy); and
5. 1 recent passport-sized photographs(Copy).

請帶備以下的文件：

1. 教育局中學學位份配-入學註冊證（正本）；
2. 出生證明書（副本）；
3. 香港身份證（副本）註：如學生所持香港身份證上出生日期下有「C」字標記（有條件限制居留），請同時帶備其他有效旅行證件（副本）；
4. 小五及小六成績表（副本）；
5. 最近的護照相片一張。

3. 專線小巴：41A。
